This form tells Intuit that you are requesting to do business with QB Services, LLC. It provides them the link between our company and yours so that if there were to be a problem or question that comes up regarding your software, they can call us to follow up and help you. Keep in mind that we already subscribe to unlimited support with Intuit so that if you are needing some problem resolved, it will most likely be best solved through us since we already know who to call.

This form needs to be filled out on your business letterhead to be valid. It will not cost you or your company anything. You may still choose to work with other companies if you like, it simply tells Intuit who you are currently working with.

To use this form, simply detach the attachment below and print it on your letterhead. You can then either scan it in and email it to us or you may snail mail it to our address on the form.
